- Job Description:
Providing one on one support to NDIS Participants aged 16 to 24 years by working alongside them to build capacity around life and employability goals.
Travelling to meet Participants on a weekly basis in the following catchment areas of: Bendigo, Maryborough, Rochester, Castlemaine, Echuca and Kyabram.
Working with Participants in conjunction with Allied Health professionals, NDIS Support Coordinators, Support Workers and State Department Case Managers.
Encouraging and assisting participants to engage in their local communities, access volunteer work, work experience and complete courses; building towards sustainable employment.
Working ‘flexibly’: on the road, visiting participants at home, in public spaces and workplaces.
Writing progress reports to provide evidence if required, another year of funding to continue supports in the next NDIS plan
