- Job Description:
Provide assertive outreach to young people and their families and developing meaningful engagement
Assess the holistic needs of the young person and their families – with a particular focus on the barriers to education re-entry (secondary is the preference.)
Establishing a responsive care team inclusive of in-kind services, development of a care plan and an education re-engagement plan and coordination of care team and capacity building with education settings for student re-entry
- Requirement:
Qualification or equivalent in Youth, Social Work or Community Welfare (Diploma +) and experience in client service functions
Ability to rapidly establish effective relationships with young people, families. schools and points of referral
Ability to form a sound ecological perspective of client circumstances, with minimal information to rapidly determine supports using appropriate tools
Understand the purpose and expectations of the Victorian Child Safe Standards and a demonstrated commitment to contributing to a child safe organisation in both practice and culture.
