- Job Description:
This role will assist clients improve their housing, general and mental health situation by:
Operating within a case management framework
Developing, implementing and reviewing case plans
Providing financial assistance via flexible care funds
Advocate for residents to strengthen links with relevant services
- Requirement:
To be successful in the role, you will have:
Formal qualification in social work, welfare or similar
Understanding of case management
Experience providing support and crisis intervention
Comprehensive understanding of the Active Service Model and HACC diversity framework
Skills in assessment, monitoring and review
