- Job Description:
provide 1:1 support to consumers in their own home or community
Provide support to assist consumers to work towards their individual goals
Assist consumers with their development of daily living skills
Provide crisi support and intervention
Increase self-management, relationship and coping skills for consumers
Meet the individual needs according to an established behavioral support plan
Develope professional relationships with consumers, their families and stakeholders
Provide accurate and timely case notes so that others can provide a consistent experience for the individual
- Requirement:
Ability to work both autonomously and within a team
Are emotionally resilient and non judge-mental
Confident to be assertive when required, transparent and patient
Effectice listening skills & an adaptable communication style
Capable to advocate for others and build support networks
Experience within a mental health, dual diagnoses and AOD an advantage
Understanding of behaviours of concern, experience also an advantage
Prior worked or lived experience supporting people with complex mental health where challanges behaviours are common
