- Job Description:
Assessments of individualis with a Mental Health Nurse to determine stability for PARC
Deliver evidence-based group content that is curated to the cohort of the ever-changing particpant group
Develop evidence-based care plans that are strengths-based with a holistic wellness focus
Develop collaborative risk management and mitigation plans
Enhancing and promoting links with natural supports in the community
Providing accurate and timely case notes so that colleagues and stakeholders can provide a consistent experience for clients
Empower people to make connections in the community
Develop recovery goals, implement steps to achieve set goals
Monitor and track the progress of goals
Support planning and review, direct support and exit planning
- Requirement:
Demonstrated flexibility, patience, initiative and emotional resilience
Capability to advocate for others and build support networks
Prior experience in mental health support or working with those who are disadvantaged in the community
Fortnightly Reflective Practice led by a Senior Psychologist
