- Job Description:
Supporting and facilitating access to accommodation on discharge from Maroondah Hospital.
Networking and collaborating with clinical services, homelessness services, public and private housing providers to ensure best outcomes for clients of the program as well as improving referral pathways between the services.
Developing systems to support residents in securing long term housing and connect with support services in the community.
Short-term intensive mental health support as well as advocacy and financial assistance to maintain existing accommodation.
Providing referrals to clinical and community services to help prevent relapse and readmission to hospital.
Support to address longer term housing plans and applications to the Office of Housing for Public and Social Housing.
This position involves travel - including to Box Hill and Maroondah Hospital.
Demonstrated Previous experience working within the housing/homelessness services.
Previous experience working in the mental health field and the recovery model.
Tertiary qualification in Mental Health and or Community Health, or a related discipline.
An understanding of referral pathways and housing networks.
Effective communication and natural relationship building skills.
Ability to think and act calmly and to deal sensitively with distress and unpredictable behaviour.
Proven networking and resourcing skills.
Demonstrated mediation and advocacy skills.
Demonstrated ability to write reports and case notes.
