$41 - $45 per hour + Super + Generous Salary Packaging benefits of up to $18,550
Collaboratively supporting the target cohort to gain access to supports by engaging with key stakeholders to establish robust referral pathways.
Educating key stakeholders about NDIS evidence/entry requirements, supports and services available, secondary consultations.
Supporting the person to liaise with health professionals to collect the evidence they need to make an NDIS access request or with consent undertake this activity on their behalf.
Liaising with NDIA to submit the Access Request Form and depending on the outcome, may follow up with submitting a review or an appeal.
There will be a requirement to meet clients if needed (particularly at Independent Assessments for Planning Meetings).
- Requirement:
As the Access Worker, you will have excellent administration, interpersonal and communication skills to provide support in assisting vulnerable people with Mental Health illness. You will bring a sound understanding of mental illness, psychiatric disability, and the effects on people's social relationships, independent living, daily needs, physical health, vocational and employment needs. Knowledge of NDIS is desirable.
