St. Vincent's Mental Health (SVMH) includes an adult area mental health service for the inner city areas of Yarra and Boroondara. The Victorian suicide prevention (HOPE) team provides follow up community support and linkage to appropriate suicide prevention services.

To have been/be a Carer or immediate family member of someone commonly acknowledged as being a Consumer, and demonstrated capacity to skilfully use your knowledge and experience to support members of family/carer/support networks
An understanding of the diversity of the cultural and linguistic backgrounds of our community, and the impact this has on Consumers', families' and Carers' experience of mental illness
Highly developed interpersonal and communication skills, including problem solving ability and conflict resolution skills, as well as sound oral and writing skills
Current National criminal history check, or willing to obtain
- Requirement:
Provide a high level of support to the individuals in the consumers personal support network
Engage with personal support networks to assist them to identify their own support needs and focus on their well-being consistent with Hospital's Family Inclusive Practice Policy and relevant legislation
Share skills and learnings in a way that brings hope, and supports and empowers personal support networks to move forward in their lives
Assist personal support networks to engage in problem-solving and gain more confidence in self-advocacy
Actively participate in HOPE referral discussions, clinical review and other relevant meetings, amplifying the carer perspective from a place of lived experience within multi-disciplinary team
Enhance effective communication between personal support networks and the program to promote collaborative treatment planning with and/or for consumers.
Where practicable facilitate peer-led groups for the family members/ friends/ supporters of HOPE consumers to model recovery, resilience and hope, or refer supporters to a service that can provide group support.
Refer personal support networks to community programs (including the Carer Peer Support Workers based at the hospital's community clinics), education and support programs as appropriate
